Personal Injury & Lien Based Exams
For patient’s completing their exams on a lien basis, their attorney MUST send us the following:
- 1. Doctor’s or Attorney’s office must fax or email the following:
- a. Doctor’s referral
- b. Patient Demographic information
- c. Attorney’s contact information
- 2. Valley MRI will forward the lien to the attorney’s office for signature and contact the patient for scheduling.
- 3. Patient will sign the lien on the day of the appointment.
- 4. Radiologist will interpret the images and generate the report.
- 5. Results will be faxed to the patient’s referring doctor and attorney.
Cash pay exams
For patients opting to pay cash for their exam, the cost of each exam will be quoted at the time of scheduling. For inquiries, contact our billing department at (209)467-1000.
Patients always have the option to pay cash and opt out of going through their insurance, but must be aware that they are not entitled to a refund from the imaging center if they decide to file a claim through their insurance after they’ve had the exam. A receipt will be provided at the time of service.

What to bring to your appointment
Always remember to bring your insurance card and some form of photo ID. If the doctor has instructed you to hand carry your exam orders, it is very important that you do not forget to bring them to your appointment. Without the doctor’s written orders we will not be able to see you. Please arrive 15 minutes before your scheduled exam time.
Request Medical Records
Walk-in anytime Monday through Friday from 8:30am-5:00pm for a copy of your medical records to take to your doctor. We will need to see a valid photo ID to verify you (the patient) before releasing any medical records. Wait time is approximately 10 minutes.

General Patient Instructions
PLEASE BRING YOUR INSURANCE CARD AND PHOTO I.D.
Plan to arrive 15 MINUTES BEFORE your scheduled appointment.
BRING any previous X-Rays or other imaging related to your current exam.
Take prescribed medication according to your physician’s instructions.
All patients requiring either IV or oral contrast should fast for at least 4 hours prior to their exam.
Diabetic patients should also fast for 4 hours prior to their exams.
Type II diabetics should skip their morning dose of oral medication Insulin dependent diabetics should consult their physician regarding their insulin dose prior to their exam.
Dialysis patients should plan to be dialyzed within 24 hours following IV contrast.
Patients receiving sedation should arrange for transportation to and from the center.
Mammogram patients: Please do not wear any deodorants, lotions or powder on the day of your exam. Also, please do NOT wear a dress or one-piece as you will be asked to change into a ½ gown.
